Аннотация

Noalkogol jigar yog'li kasalligini (NJYK) erta va kam invaziv usullar bilan tashxislash zamonaviy gepatologiyaning hal etilmagan muammolaridan biri bo'lib qolmoqda. Ushbu maqolada qon zardobidagi seramid ko'rsatkichining NJYKni tashxislash va prognozlashdagi diagnostik-prognostik ahamiyati ROC-tahlil hamda binar logistik va ordinal regressiya modellari asosida baholangan. Tadqiqotga 84 nafar shaxs (nazorat guruhi - 29, steatoz guruhi - 28, steatogepatit guruhi - 27 nafar) jalb etilib, seramid darajasi HPLC-MS usuli bilan aniqlangan. ROC-tahlil natijalariga ko'ra, seramid NJYKni aniqlashda yuqori diagnostik aniqlikka ega bo'ldi (AUC=0,932; cut-off ≥11,71 u.b.; sezuvchanlik 81,8%, spetsifiklik 93,1%). Binar logistik regressiyada seramid yakka modelda mustaqil ahamiyatli prediktor bo'lgan (OR=1,232; 95% CI: 1,092-1,391; p=0,0007), yosh, jins va tana vazni indeksi (TVI) qo'shilgandan so'ng modelning to'g'ri tasniflash darajasi 96,4% gacha oshgan. Ordinal regressiya tahlilida seramid steatoz (OR=1,166; p=0,0026) va fibroz (OR=1,042; p=0,0030) darajalari bilan mustaqil bog'liq prediktor sifatida saqlanib qolgan. Olingan natijalar asosida seramid, TVI va lipid spektri ko'rsatkichlarini birlashtirgan amaliy diagnostik-prognostik model taklif etildi.
